皮肤交界性平滑肌瘤的复发与恶性转化

目的探讨皮肤交界性平滑肌瘤的复发与恶性转化。方法复习2例皮肤交界性平滑肌瘤及其复发恶性转化的临床病理特征、免疫组化表型及相关文献。结果2例病变位于下肢和额部，肉瘤组织平滑肌标记阳性。结论少数病理诊断为皮肤平滑肌瘤的病例，虽然浅表且组织形态良性，但其生物学恶性潜能尚不能确定，部分病例复发后可转变为典型的平滑肌肉瘤。     

脑梗死后再发缺血性卒中与脑出血患者微血管病变及预后的比较

【目的】探讨脑梗死后再发缺血性卒中与脑出血患者的临床特点、影像学上微血管病变及预后的异同。【方法】连续收集急性复发性卒中的住院病例（初发事件为脑梗死，复发事件包括脑梗死或脑出血），详细记录临床资料。观察脑内微出血及自质病变的严重程度，随访有无严重血管事件及死亡的发生。【结果】175例再发脑梗死患者。19例复发事件为脑出血的患者进入本研究。155例（88．6％）再发脑梗死患者，以及18例（94．7％）脑出血患者存在自质病变。77例（44％）再发脑梗死患者和16例（84．2％）脑出血患者存在脑内微出血。脑出血组自质病变的严重程度及微出血的数目均明显高于再发脑梗死组（P=0．033，0．016），脑出血组的死亡率及再发脑出血的危险性明显增高（P=0．034，0．036）。【结论】严重的脑自质病变及微出血的出现与脑梗死后再发脑出血有关。多发微出血提示出血风险增高。如复发事件为脑出血，则其死亡率及再次复发脑出血的风险均明显增加。     

糖尿病肾病临床研究述评

本文从中医病名、病因病机、诊断标准、临床分期、疗效评定、辨证论治、临床科研七个方面对糖尿病肾病的研究进行了述评。指出：糖尿病肾病的中医病名、诊断、分期及疗效评定标准亟待统一；立足于临床实际，参考现代医学有关认识，应用ＤＭＥ方法，开展糖尿病肾病临床和实验研究，深入探讨其病因病机和证治规律，在中医基本理论指导下，大胆创新，是提高临床疗效的关键；分期辨证论治较为符合疾病的发展规律，便于寻找防治糖尿病肾病的有效方法。     

影响部队汽车驾驶员心身健康水平和行车安全的相关因素研究

对部队汽车驾驶员203名的心身健康和行车安全的相关因素研究表明:驾驶员可发生心身障碍,尤其眼和耳、呼吸系统,骨胳肌肉系统及抑郁等为著。其影响因素,主要有负性精神紧张总值(-Lcu)、个性的精神质、神经质、掩饰性、内向倾向、开车公里数、开车年限及精神疾病家族史。某些心身问题、神经质、掩饰性较高、年龄偏大、开车公里数多及开车年限长等与事故差错的发生有关。     

Global environmental change: preventive measures to reduce the impact on human health

Awareness of the magnitude of worldwide climate change and theloss of biological diversity that may occur as the result ofpresent human activities is increasing. However, much needsto be done to reverse current trends and prevent unprecedentedglobal environmental changes which are expected to produce irreversibleresults, and to have profound implications for the health ofindividuals. This paper briefly reviews what is known aboutthe global environmental changes that are taking place and theirpredicted impact on human health. Some of the preventive measuresand activities in which public health professionals need toassume leadership roles in the critical years ahead are discussed.Our hope is to enliven a much needed dialogue among health professionalson the crucial issue of environmental survival in which preventionmay be the only viable solution.     

Malignant Warthin''s tumour: a case study

Malignant transformation of a benign Warthin's tumour (adenolymphoma) is extremely rare. The light microscopic and ultrastructural features of an adenocarcinoma arising in a Warthin's tumour in the parotid gland are described. Light microscopy demonstrated a transition zone from the benign to the malignant component, and the malignant cells revealed oncocytic features by both light- and electronmicroscopy.

目的 探讨乳腺导管内乳头状瘤(intraductal papillomas，IP)癌变的临床表现、生物学行为、诊断、治疗及预后。方法 对1980年1月至2001年12月间7例IP癌变病人临床资料进行回顾性分析。结果 7例IP癌变占同期113例IP的5．19％，术前均未能确诊，5例术中冰冻考虑IP癌变。结论 IP有癌变潜能，其癌变者术前确诊困难，术中冰冻对诊断治疗意义重大，应重视术后随访。     

Diversity in Clinical Psychology

Clinical psychology is not as diverse as society, and the generalizability of clinical psychology to diverse groups has largely been untested. Some progress has been made in increasing the number of ethnic minorities receiving PhD degrees in clinical psychology and in increasing ethnic minority representation in clinical trials. Nevertheless, clinical psychologists and clinical psychology research are not diverse. A stages of change model may be useful in motivating clinical psychologists to diversify. A future scenario involving inertia and a second involving change are offered.
